What is the Isences® TMD Physio Care™?
Isences® TMD Physio Care™ is a medical-grade jaw rehabilitation device designed for physiotherapists and rehabilitation professionals managing temporomandibular disorders (TMD), and functional jaw limitations.
The device incorporates inflatable functional elements that guide the mandible through a controlled physiological biaxial shear–rotational movement pattern, supporting restoration of mobility while helping to limit excessive loading of the temporomandibular joint (TMJ).
Clinical Application
The device is designed to support:
- Mobility training
- Neuromuscular re-education and coordination
- Controlled strengthening of masticatory muscles
- Functional rehabilitation of chewing efficiency
When used as part of a structured rehabilitation program, it may support improved jaw function and reduction of muscle-related discomfort.
Extended Rehabilitation Use
Isences® TMD Physio Care™ may also be applied in orofacial rehabilitation contexts, including:
- Supportive therapy for post-stroke dysphagia
- Improvement of orofacial muscle coordination
- Facilitation of safer swallowing patterns within multidisciplinary care
Clinical Positioning
Isences® TMD Physio Care™ provides a controlled, progressive interface between passive mobility and active strengthening, making it suitable for integration into:
- TMD physiotherapy protocols
- Jaw rehabilitation programs
- Orofacial and functional recovery pathways
Summary
A bioengineered, clinically oriented solution for guided jaw rehabilitation, supporting mobility, control, and functional recovery through controlled, joint-conscious movement.
Intraoral Dimensions & Adaptability
The device is designed to enable controlled insertion and progressive mandibular expansion in patients with moderate functional limitation.
- Reduced-volume insertion:
Withdrawing approximately 20 ml of air allows insertion at a minimal interincisal opening of ~17 mm - Progressive expansion range:
Controlled inflation up to ~ max. 17 ml of air supports gradual increase in mouth opening up to ~36 mm
Performance Note
Maintains functional integrity for >3000 exercise cycles when used within recommended air volume parameters
Values are approximate and dependent on individual anatomy and clinical progression
Functional & Dynamic Force Characteristics
The Medium-Force Jaw Exerciser™ provides moderate, adjustable mechanical loading, enabling progression from mobility-focused to strength-oriented rehabilitation.
Mandibular Movement Amplitude
- Supports controlled jaw-opening exercises up to ~19 mm functional amplitude
→ Facilitates expanded range training with maintained control
Stretching Force - Mobility Training
- Generates up to ~24 N of stretching force
→ Supports advanced soft tissue extensibility
→ Addresses residual stiffness and adaptive shortening
Strengthening Force - Stability Training
Provides resistance up to ~24 N, depending on:
- Air volume
- Bite amplitude
→ Enables moderate-load isometric strengthening
→ Supports improvement of muscle performance and endurance
Load Modulation
Resistance is dynamically adjustable via:
- Air volume (primary control variable)
- Occlusal compression amplitude (secondary variable)
→ Allows graded, patient-specific dosing of mechanical load
Performance Note
Maintains consistent mechanical performance for >3000 cycles under recommended loading conditions
Mobility Training - Gentle Stretching
Purpose:
Support gradual improvement in jaw opening, reduce stiffness, and restore comfortable movement.
Step-by-Step
Sit in a relaxed position
Keep your head upright and shoulders relaxed
Place the device
Position it gently between your back teeth (molars)
Adjust for insertion if needed
Remove some air to reduce size if opening is limited
Let the device adapt
Allow it to expand naturally to your current jaw opening
Gradual inflation
Reconnect the syringe and slowly add air every 3–5 seconds, up to a maximum of 17 ml, or until a comfortable stretch is reached.
Reach a gentle stretch
Stop when you feel a moderate, comfortable stretch (never pain)
Hold the stretch
Close the one-way stopcock and hold for 10–15 seconds
Release slowly
Open the stopcock and release air before removing the device
Recommended Use
- Repetitions: 3–5 per session
- Frequency: 3–5 sessions daily
- Intensity: light to moderate
(approximately 3/10 effort on a subjective scale)
Progression:
Increase hold time to 15–20 seconds after 1–2 weeks if comfortable
Safety Note:
- Stretch should remain comfortable, not painful
- Avoid sudden or forceful movement
- Reduce intensity if soreness occurs
Stability Training - Controlled Strengthening
Purpose:
Improve jaw muscle coordination, control, and joint stability.
Step-by-Step
Set air level
Inflate to approximately 5 ml
Close the stopcock
Maintain internal pressure
Apply controlled bite
Gently bite with small movement (~4–7 mm)
Hold contraction
Maintain for 5 seconds
Relax slowly
Release in a controlled manner
Progression
- Increase air to ~7 ml for higher resistance
- Maintain smooth, controlled movement at all times
Recommended Use
- 5–10 repetitions per session
- 1–2 sessions per day
- Intensity: moderate (15–30% effort)
Safety Note
Movements must be smooth and symmetrical
Avoid clenching or excessive force
Stop if pain, clicking, or locking occurs
Balancing Flexibility and Strength
Purpose
Maintain a healthy balance between mobility and stability for efficient jaw function.
How to Apply
Follow this sequence in each session:
Start with mobility - stretching
Improves range of motion and reduces stiffness
Then perform stability - strengthening
Helps stabilize the jaw within the new movement range
Example Session
Gentle stretch × 3 repetitions (5–7 seconds each)
Isometric resistance × 5 repetitions
(5 seconds each)
Relaxation phase
Close the jaw gently and perform light facial massage for 1–2 minute
Key Tips
Keep all movements controlled and pain-free
Focus on smooth, balanced motion
Progress gradually — not forcefully
General Safety & Customization
Follow These Core Rules
1. Keep all movements pain-free Only perform exercises within a comfortable range → If you feel pain, stop immediately
2. Monitor your response after use
After each session:
- Mild temporary sensation is acceptable
- Pain, swelling, or stiffness should not last longer than ~6 hours
→ If symptoms persist, reduce intensity or stop use
3. Maintain correct posture
- Sit upright and relaxed
- Keep your head in a neutral position
- Avoid leaning forward or clenching
4. Use a calm environment
- Perform exercises in a quiet, relaxed setting
- Avoid distractions that may increase tension
5. Progress gradually
- Increase resistance or jaw opening step by step
- Do not rush progression
- For best results, progress under professional guidance
Medical Conditions & Recovery Situations
⚠️ Do not use the device unless cleared by a qualified clinician if you have
- Recent injury or surgery (jaw, face, TMJ, or dental procedures such as implants)
- Healing restrictions (fixed or wired jaws, post-surgical recovery phase)
- Serious medical or structural conditions(e.g., tumors in the jaw/face area, severe bone fragility, TMJ ankylosis)
- Neurological or unexplained symptoms
(persistent pain, numbness, loss of control, or unusual jaw movement)
Important
If you are unsure, consult a healthcare professional before use.
